          Brookings Neuropsychology Services will be closing it's                       Brooking office on March 31st 2015. 

        Dr. Shaffer will continue to accept referrals in Brookings 
       until that time, and will subsequently relocate to Avera 
       Behavioral Health Outpatient Services in Sioux Falls. 
       Appointments in Sioux Falls will likely be available in late 
       April or early May of 2015.

     Dr. Shaffer  will continue to accept referrals for 
    neuropsychological, psychological, psychoeducational, and 
    forensic testing and evaluation in Sioux Falls. Referrals,       
    requests for further information, or appointments    
    should be directed to Avera Behavioral Health 
    Outpatient Services at (605) 322-4079 after 4/15/2015.
